ACA-IgA (Anti-Centromere Antibody Immunoglobulin A) is a specialized autoimmune blood test used to detect antibodies that target centromere proteins within the nucleus of cells. The presence of ACA-IgA is commonly associated with certain autoimmune connective tissue disorders, particularly limited cutaneous systemic sclerosis (CREST syndrome) and other autoimmune conditions. This test plays an important role in early diagnosis, disease classification, and long-term patient management.

What is ACA-IgA?

Anti-Centromere Antibodies (ACA) are autoantibodies directed against centromere components of chromosomes. While ACA-IgG is more commonly tested, ACA-IgA provides additional diagnostic value, especially in mucosal immune responses and disease monitoring. Elevated ACA-IgA levels may indicate immune system dysregulation and ongoing autoimmune activity.

Why is the ACA-IgA Test Done?

The ACA-IgA test is recommended when patients show symptoms suggestive of autoimmune or connective tissue diseases. Doctors may advise this test to:

  1. Support diagnosis of systemic sclerosis (limited type / CREST syndrome)

  2. Evaluate unexplained Raynaud’s phenomenon.

  3. Assess autoimmune involvement in chronic inflammatory conditions

  4. Aid in differentiating subtypes of connective tissue disorders

  5. Monitor disease progression or immune activity

Symptoms That May Require ACA-IgA Testing

  1. Cold-induced color changes in fingers or toes

  2. Skin tightening or thickening

  3. Joint pain and stiffness

  4. Difficulty swallowing

  5. Chronic fatigue

  6. Unexplained autoimmune symptoms

How is the ACA-IgA Test Performed?

The ACA-IgA test is a simple blood test. A blood sample is collected from a vein and analyzed using advanced immunological techniques such as ELISA or immunofluorescence assays. No special preparation or fasting is usually required unless advised by a physician.

Understanding ACA-IgA Test Results

  1. Negative Result: Indicates absence or normal levels of ACA-IgA antibodies.

  2. Positive Result: Suggests an autoimmune process, often linked with limited systemic sclerosis or related conditions. Clinical correlation is essential, as results must be interpreted alongside symptoms and other investigations.

Clinical Importance of ACA-IgA

ACA-IgA testing helps clinicians:

  1. Detect autoimmune disorders at an early stage

  2. Distinguish between limited and diffuse systemic sclerosis

  3. Predict disease course in certain autoimmune conditions

  4. Plan appropriate treatment and monitoring strategies
